Old Faithful Isle is a large island located within the The Wilds at coordinate M-4.

The island is multi-leveled, with most of its shoreline being replaced by tall cliffs. Small beach landings can be found to the north and south with a larger beach to the East protected by cannon towers. There are several abandoned fortified structures built around the island.

The island features:

3 Ammo Chests
- On the upper level, just outside the fort gatehouse among some barrels by the stone archway.
- Next to the Summoning Altar within the fort on the upper east level of the island.
- On the platform of the fort wall overlooking the north-west shoreline.
3 Campfires
- To the west near a hanging cage, against a rocky cliff facing North.
- In the cave in the southern section of the east beach.
- On the south beach by the water.
2 Cannons, both facing east on towers by the east beach.

- The Fate of the Morningstar
Glitterbeard's Journal: "Wrecks of the Lost", next to some barrels at the top of the highest peak.

The following Tall Tale objectives take place on this island:

The Fate of the Morningstar
Ghostly memories, beginning at the south beach, leading to the fates of Dinger, Eli, and Fontaine. Possible location for the three Skeleton Chests & matching Skeleton Keys, led to using the Enchanted Lantern and following the foot print clues & ghostly memories starting from the south beach. Journal: "Still No Luck", found near a skeleton's remains atop the highest peak of the island to the north-west. Journal: "A Ghost Watch", found on a rock near a white flag inside a cave by the south beach. Journal: "Giving Up , found inside a tower to the south-east. Journal: "The fright of my life", found at the western tip of the island resting on a Kraken skull.

Possible location to encounter Graymarrow.
